Delivery System Reform Incentive Payment Program (DSRIP)

DSRIP initiatives provide states with significant funding that can be used to support hospitals and other providers in changing how they provide care to Medicaid beneficiaries. DSRIP´s purpose is to fundamentally restructure the health care delivery system by reinvesting in the Medicaid program, with the primary goal of reducing avoidable hospital use by 25% over 5 years. Up to $6.42 billion dollars are allocated to this program with payouts based upon achieving predefined results in system transformation, clinical management and population health.

The members of Excelsior Medical IPA have joined ACP, our partner in DSRIP. ACP, or Advocate Community Providers, Advocate Community Providers is a new community-based network of over a thousand physicians poised to transform health care delivery across New York City.

Patient-Centered Medical Home (PCMH)

The patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into “what patients want it to be.” Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.

The primary care medical home is accountable for meeting the large majority of each patient’s physical and mental health care needs, including prevention and wellness, acute care, and chronic care.
